Xylometazoline and ipratropium: Effective combination

A combination of two topical agents, to relieve patients’ worst cold symptoms at once:

Xylometazoline hydrochloride (0.05%):

  • A sympathomimetic agent, provides rapid and long-lasting relief (within 5-10 minutes and for up to 6-8 hours) of nasal congestion3-5
  • It improves nasal airflow, reduces mucus hypersecretions3 and relieves sinus pressure6-9

Ipratropium bromide (0.06%):

  • An anticholinergic agent that reduces watery discharge from nasal mucosa (rhinorrhoea) and lessens sneezing2,4,10,11

Otrivin Dual Relief product details

  • Active ingredients:1

    Nasal spray solution (metered-dose spray):

    • 1 ml contains 0.5 mg xylometazoline hydrochloride and 0.6 mg ipratropium bromide
    • 1 spray delivers 0.140 mL of solution, containing 0.084 mg of ipratropium bromide and 0.07 mg of xylometazoline hydrochloride

    Inactive ingredients:1

    Disodium edetate, glycerol (85 per cent) (Ph. Eur.), hydrochloric acid (for pH - adjustment), sodium hydroxide (for pH - adjustment), and purified water.

  • Dosing instructions:1

    • For adults only ≥18 years of age
    • Leave at least 6 hours between doses
    • Treatment duration should not exceed 7 days

    Method of administration:1

    • Before first use, the pump should be sprayed several times into the air until an even spray is produced
    • If the spray has not been used for a few days, or there is not an even spray, the initial process should be repeated
    • If the full spray is not administered, the dose should not be repeated

    How to use:1

    • Be careful not to spray into the eyes
    • Clear the nose
    • Hold the bottle upright with your thumb under the base, and the nozzle between two fingers
    • Lean forward slightly and insert the nozzle into a nostril
    • Spray and breathe in gently through the nose at the same time before repeating in the other nostril
    • Clean and dry the nozzle before replacing the cap immediately after use
    • To avoid possible spread of infection, the spray should only be used by one person
    • Always read the label before use
    • This product, like other sympathomimetic agents, should be used with caution in patients showing a strong reaction to adrenergic substances, such as insomnia, dizziness, tremors, cardiac arrhythmia, or elevated blood pressure
    • This product should be used with caution in patients with hypertension, cardiovascular disease, hyperthyroidism, diabetes mellitus, phaeochromocytoma, prostatic hypertrophy, bladder outlet obstruction, cystic fibrosis, monoamine oxidase inhibitors (MAOI) treatment, or who have received them in the previous two weeks
    • Patients who take tri and tetra-cyclic antidepressants or beta 2-agonists
    • Caution is recommended in patients predisposed to angle-closure glaucoma, epistaxis (e.g. the elderly), or paralytic ileus
    • As with other topical vasoconstrictors, the treatment duration should not exceed seven days; prolonged or excessive use may cause rebound congestion and/or atrophy of the nasal mucosa
    • There have been rare cases of posterior reversible encephalopathy (PRES)/reversible cerebral vasoconstriction syndrome (RCVS) reported with sympathomimetic drugs. Symptoms reported included sudden onset of severe headache, nausea, vomiting, and visual disturbances. Most cases improved or resolved within a few days following appropriate treatment. This product should be discontinued immediately and medical advice sought if signs/symptoms of PRES/RCVS develop
    • Patients should be instructed to avoid spraying this product in or around the eye. In the event of eye contact, the following may occur: temporary blurred vision, irritation, pain, or red eyes. Aggravation of angle-closure glaucoma may also develop
    • The patient should be instructed to rinse their eyes with cold water if the product gets in direct contact with the eyes and to contact a doctor if they experience pain in the eyes or blurred vision
    • Keep out of sight and reach of children
